Our goal is to provide the public with a more convenient and efficient means with which to interact. We collect information in two distinct areas: (1) Get Support creation (2) interaction with the Web site and tools. We do not collect information about you unless you voluntarily participate in an activity that asks for such information. The information requested and supplied by you through the web, is the same information requested if performed in person at a local or State agency. If personal information is requested on the web site or volunteered by you, the Health Insurance Portability and Accountability Act (HIPAA) of 1996 may protect it. Should you have any questions, contact the agency directly about the records we keep and our use of information. We also use your designated email to respond to your requests and to the issues you have identified, and/or possibly to forward your email to another agency for appropriate action.
When you complete a self-referral, you give us permission to send your information to a partnering agency. If we change the way we use any of your individually identifiable information, we will contact you either by phone or via email at the number and address designated in your referral.

Access to your information is given only to Dallas County ADRC affiliates who need to review the data as part of the program functions. As part of Dallas County ADRC’s orientation, all affiliates receive information about Dallas County ADRC’s privacy practices and their roles and responsibilities.
